    1. Applicant(s) must be able to prove their gross monthly income of Three (3) times the monthly rent. This includes tip earnings and self-employed persons, please submit copies of Three (3) recent pay-stubs, banks statements (all pages) or if self-employed submit Tax Return of prior year.

    2. Please submit a copy of each applicant’s Driver's License or State ID.

    DISCLOSURES

    The applicant agrees to the following terms and conditions.

    1) Applicant declares that the information contained herein is true and correct.
    2) Applicant authorizes an employment check,criminal background check, credit check, verification of references and current and previous landlords.
    3) Applicant hereby agrees to pay non refundable application fee and admin fee at applicant's expenses. If an applicant does not pay these fees within One (1) calendar day, the application will not be processed and this application will be automatically revoked.
    4) If application is approved, applicant agrees to execute a lease agreement within Two (2) business days and pay all required security deposit, rent holding fee and other fees (if any) within Two (2) business days.
    5) If after approval, the applicant decides to not fulfill this signed lease agreement, security deposits and rent holding deposit shall be forfeited per pursuant to the terms of the holding and fees agreement.
    6) Landlord and agent will not be bound by any representations, agreements or promises written or oral, made by landlord or agent unless contained in the lease agreement signed by landlord or landlord's agent.
    7) Applicant does hereby release landlord, agent and this company from any and all damages or liabilities which might result from the above information. Applicant releases present landlord and all previous landlords from any and all liability for any damage or injury whatsoever caused by providing information to landlord or agent regarding applicant.
    8) Applicant understands and acknowledges that a false statement made herein grounds for denial of rental to applicant. Any statement herein may be construed as a condition precedent to any binding lease agreement or contract between applicant and landlord.
    9) Approval for residency is made without regard to racem color, religion, sex, national origin, age, gender identity or expresion, familial status, sexual orientation, ancentrey or handicap.
    10. Applicant understands that applicant acquires no rights to premises until execution of lease agreement in the form submitted and deposit of holding rent, security deposit and fees (if any) described above.
